The following information must appear on all custom-mixed feed labels, and a copy must be furnished to the customer with each load. A duplicate copy must be maintained at the feed mill for sampling and inspection purposes:

• Name and address of manufacturer of the custom-mixed feed.
• Name and address of the purchaser.
• Date of manufacture.
• The product name and net weight of each commercial feed and each other feed ingredient used in the mixture.
• Adequate feeding directions.

If the custom-mixed feed is medicated, the following information must also be supplied:

• The purpose for which the medication is used (claim statement).
• The name and potency of each drug used in the feed.
• The name and level of each drug in the finished feed.
• All appropriate precautions, warnings, and withdrawal statements.
